ANGEL Production Site Request Form

Please provide the information below to have a blank ANGEL shell enabled for your class.  You MUST request a site for each course you plan to run, otherwise no site will be enabled and your students will not be imported when classes begin. 

YOU MUST FILL OUT THIS FORM ONCE FOR EACH CLASS NUMBER.  If you have more than one class of the same course, we can merge your rosters into one site for you prior to the start of term.  However, you must still indicate all class numbers individually via this form or the student enrollments will not be processed for ANGEL.

Please be sure to complete this form AT LEAST TWO WEEKS before the beginning of the term to ensure that your course is loaded and populated promptly.
