ANGEL Site Request Forms

Please select the type of ANGEL site you would like to request. 

*NOTE:  You must complete one request PER CLASS.  Please allow 1-2 weeks for request completion.

ANGEL Production Site
  Fill out this form to tell us which courses and class numbers you will be running live in ANGEL during a regular term.  You must complete this form to have a course shell created and students populated into your class when the term begins.  Sites for future terms will be created two weeks prior to the course start date.  If you need to work on your course before that time, please request a development site. 

ANGEL Development Site
  Fill out this form to have a new, blank ANGEL site set up for you to begin new content development on a new course, or work on content for next semester.
